Scope of this policy

This policy describes how information is handled when you interact with this consultation landing page. It does not apply to separate patient records maintained for people who become patients.

This form is not for medical information

The consultation and patient-resource forms collect basic contact details only. Please do not submit diagnoses, symptoms, medications, treatment history, insurance information, images, or other sensitive health information through this website.

Information we collect

Depending on the form you use, we may collect:

• Name, phone number, and email address
• Your preferred reply method and optional text-message consent
• The patient resource requested and its delivery or download status
• Submission date and time, campaign parameters, and advertising click identifiers
• Basic device, browser, security, and page-interaction information used by the deployed site

How we use information

• Respond to consultation requests and deliver requested resources
• Record text-message consent and opt-outs
• Detect abuse and operate, maintain, secure, and measure the website
• Comply with legal obligations

Optional consultation text messages

If you separately agree to receive texts, we use your mobile number and consent record for consultation-request follow-up, including automated messages, scheduling, appointment reminders, and replies. Message frequency varies. Message and data rates may apply. Reply STOP to opt out or HELP for help. Text consent is optional and is not a condition of purchasing services or requesting a consultation. You may choose a call or email instead.

We retain your affirmative consent choice, the wording version, submission time, and source page. Consent to reply to an earlier inquiry does not automatically authorize a broader messaging program. See our SMS Terms for details.

How we share information

We share information only with service providers needed to host and secure the site, deliver communications, measure performance, provide advertising measurement (Meta Pixel, only if you agree), and manage inquiries. We may also disclose information when required by law. We do not sell or rent personal information.

Mobile information, text-message originator opt-in data, and consent are not sold, rented, or shared with third parties or affiliates for their own marketing or promotional purposes. Service providers may process this information solely to deliver and support communications on our behalf.

How long we keep information

• Consultation and patient-resource requests: generally up to 12 months after the last interaction
• Private resource-download links: 14 days
• Text consent, opt-out, and limited compliance records: generally up to 4 years
• Technical and security logs: generally up to 90 days

Information may be deleted sooner or retained longer when law, security, fraud prevention, disputes, or opt-out records require it.

Your choices and requests

You may ask to access, correct, or delete information submitted through this site by emailing info@yorbalindaoms.com. We may need to verify your identity, and some records may be retained when required by law or a legitimate business need.

Security

We use reasonable safeguards designed to protect site information, but no transmission or storage method is completely secure.

Children, third-party links, and browser controls

This site is not directed to children under 13. Third-party links are governed by their own policies. Browser privacy controls may limit certain tracking; the site does not currently respond to Do Not Track signals. Use Advertising preferences on this landing page to agree to, decline, or withdraw Meta Pixel consent. The choice applies immediately, even if the browser cannot save it.

Google Maps

The landing page includes an embedded Google Map and links for directions. Loading or opening Google Maps may share technical information such as your IP address, browser or device information, and the referring page with Google. Do not enter medical details into Google Maps. Google handles its services under its own terms and privacy policy.

Meta Pixel and advertising cookies

If you choose Agree on the advertising-measurement prompt, this landing page may load the Meta Pixel (dataset ID 2217918019068989) after production transmission is enabled. The pixel is not loaded until you agree, and it is not sent to Meta while production transmission remains off. If you choose Decline or Withdraw, later Meta events from this page are not sent.

We configure the pixel without automatic advanced matching and without automatic event detection. We do not send your name, email, phone number, or form answers as pixel parameters. Hashing is not used as a substitute for withholding that information. We do not claim that Meta cannot see health-related language in the page URL or referrer.

If you agree, Meta may still receive technical information that the browser pixel collects automatically. This can include the live page URL and referrer, advertising click identifiers already present in the link, browser and device type, IP address as processed by Meta, and advertising cookies. Campaign parameters remain in the page URL so existing advertising tags can attribute the visit, and may include health-related campaign language. Withdrawing consent stops later Meta events from this page, but cannot take back a PageView already sent.
