Scope of this policy

This policy describes how information is handled when you interact with this consultation landing page. It does not apply to the separate patient records that our practice maintains for people who become patients, which are governed by the practice's own record-keeping obligations.

This form is not for medical information

The website consultation and patient-resource forms are intended only to collect basic contact details so a team member can reach out to you or deliver information you requested. Please do not submit diagnoses, symptoms, medications, treatment history, insurance information, images, or any other sensitive health information through this website. If you need to share health information, please wait until we contact you and can do so through an appropriate channel.

Information we collect

Through the consultation and patient-resource forms, we may collect:

  • First name and last name
  • Telephone number and country code
  • Email address
  • Which patient resource you requested and whether it was delivered or downloaded
  • Whether you opted in to text messages, and the date and time of that choice or any later opt-out
  • The date and time your submission was sent
  • Referral or campaign parameters (such as UTM tags) if they are present in the link you used to reach the site

When you visit the site, our hosting and security systems may also collect basic technical information such as your IP address, the type of device and browser you are using, and general page interactions. We only collect these items to the extent the deployed site actually uses them.

How long we keep information

  • Consultation request information: generally kept for up to 12 months after your last interaction if you do not become a patient.
  • Patient-resource request information: generally kept for up to 12 months. Private resource-download links expire after 14 days.
  • Text-message consent, opt-out, and limited compliance records: generally kept for up to 4 years after your last message or opt-out, or longer only if the law requires it.
  • Technical and security logs: generally kept for up to 90 days.
  • If you become a patient, information needed for your care may be transferred to the practice's regular patient-record system, which is kept under the practice's separate record-retention obligations. The landing-page database is not intended to serve as a patient chart.
  • We may delete information sooner when appropriate. We may also keep information longer when necessary for legal, security, fraud-prevention, dispute, or opt-out-record reasons.

Your choices and requests

You can ask us to access, correct, or delete the personal information you submitted through this website by emailing info@yorbalindaoms.com. We may need to verify your identity before responding, and some information may be kept when the law or a legitimate business reason (such as an opt-out record) requires it.

Security
